• 1- Print out of the application filled.
• 2- Two Photograph of the child.
• 3- Self attested photocopy of Date of birth certificate.
• 4- Self attested photocopy of Residential Proof.
• 5- Self attested photocopy of documentary proof for E.W.S ( income certificate ) or DG ( Caste certificate ). Which ever applicable.
• 6- Aadhar card of the child.
